NEW YORK (Reuters Health) May 22 - Early intensive insulin therapy in newly diagnosed type 2 diabetics can enhance beta-cell function and lead to extended glycemic remissions, according to a report in the May 24th issue of The Lancet.
Prior research has suggested that such therapy could positively influence the natural course of type 2 diabetes, Dr. Jianping Weng, from the Third Affiliated Hospital of Sun Yat-Sen University in Guangzhou, China, and colleagues note.
To investigate, the research team assessed the outcomes of 382 patients who were randomized to receive treatment with insulin given either in a continuous subcutaneous infusion or as multiple daily injections, or standard oral agents.
Treatment with the assigned agent was stopped after normoglycemia was achieved for 2 weeks. The subjects then switched to diet and exercise management alone.
With early intensive insulin therapy, nearly all patients achieved target glycemic control within 4 to 6 days, the report indicates. With oral drug therapy, by contrast, subjects often needed 9 days or longer to achieve control.
The 1-year glycemic remission rates with the insulin infusion and injection protocols were 51.1% and 44.9%, respectively. With oral drug therapy, the glycemic remission rate was just 26.7%.
HOMA B, which is a measure of basal beta-cell function, and insulin response results indicated that the intensive insulin regimens significantly improved the recovery and maintenance of beta-cell function.
In a related editorial, Drs. Ravi Retnakaran and Daniel J. Drucker, from the University of Toronto, comment, "Although the relevant biological mechanisms and target tissues contributing to preferential improvement in beta-cell function remain unclear, these data suggest that the use of intensive insulin therapy early in the course of type 2 diabetes warrants further clinica investigation."
